A 42 yrs old male with fever and vomitings.  8 june 2023 Hi iam Likitha 3rd semester student. This is an online elog book to discuss our patients health data after taking his consent, this also reflect my patient centered online learning portfolio.  Case discussion: Date of admission:7 june 2023 A 42 yrs old male resident of chennaram came to medicine OPD with Chief complaints of: •Fever since 3 days • vomiting on 6 june 2023 at 1pm History of present illness: Patient was apparently asymptomatic 3 days back then he developed fever which is insidious, gradually progressive. Fever is associated with chills. He complained decreased urine output since 5 days.  Vomiting is non bilious, non projectile, non blood stained, contains food.  No H/O:loose stools, abdominal pain.  History of past illness: In feb 2023 patient developed abdominal tightness & chest discomfort he went to nearby hospital where he was diagnosed has antral gastritis.  1 month back he was diagnosed has type

This is an online elog book to discuss our patients health data after taking his consent, this also reflect my patient centered online learning portfolio.  Case discussion: Date of admission:6 june 2023 A 40 yrs old female resident of miryalaguda came to medicine OPD with  Chief complaints of: •SOB since 4 days.  •Fever, vomitings, pain in the abdomen, diarrhea and decreased urine output since 3 days.  History of present illness: Patient was apparently asymptomatic 4 days back, then she developed SOB since 4 days , SOB is exacerbating while exertion and relieved by resting after one day she developed fever, vomitings and diarrhea.  •Vomitings : 3-4 episodes per day, non projectile, non bilious, contain food particles and water •Diarrhea:4-5 episodes per day                 Watery stool                  No blood observed in the stool •Pain in the abdomen is continuous and non radiating

A 48 yrs old female with abdominal distension,and decrease in urine out put. 6/6/23 Hi iam Likitha 3rd semester student. This is an online elog book to discuss our patients health data after taking his consent, this also reflect my patient centered online learning portfolio.  Case sheet: A 48 yrs old female, resident of panagallu came to medicine OPD with Chief complaints of: •Abdominal distension since 6 days •SOB since 5days •B/L lower limbs swelling •Decrease in urine output since 3days History of present illness: Patient was apparently asymptomatic 6 days back, then she developed  abdominal distension followed by SOB, pedal oedema and oliguria. Pedal oedema is bilateral , which is incidious in onset and pitting type.  History of past illness: •K/C/O:DM( since 10 yrs)  NK/C/O:HTN/TB/Asthma/CAD/epilepsy •she showed similar symptoms of abdominal distension, SOB, pedal oedema &decreased urine output 2 yrs back. This is an online elog book to discuss our patients health data after taking his consent, this also reflect my patient centered online learning portfolio.  Case sheet: A 40 yr old man, lorry driver by occupation, resident of survellu came to medicine OPD with Chief complaints of: •Abdominal distension since 4-5 days.  •Abdominal bloating since 4-5 days.  • Shortness of breath since 5days. •yellow discoloration of eyes since 5 days.  •Vomiting since 3 days.  •B/L lower limbs swelling since 15-20 days.  History of presenting illness: Patient was apparently asymptomatic 15 days back , then he developed B/L lower limbs swelling since 15 days then followed by shortness of breath, abdominal distension, yellow discoloration of eyes since 5 days.  History of past illness: K/C/O:DM(since 5 yrs)  NK/C/O:HTN/TB/ASTHMA/CAD/EPILEPSY Personal History:
